  1. You are required to have a signed Controlled Substance Agreement in your Medical Record if receive Narcotic Medications. Southwest
  2. Medical Clinic will not release any prescriptions for narcotics without a patient completing this agreement.
  3. In addition, it is our policy at Southwest Medical Clinic not to call in any Class II Narcotic medications to any pharmacy at any time. This is not only a policy but a law regulated by the DEA (Drug Enforcement Administration). For example, the following medications will not be called in to any pharmacy:

Duragesic Patches, Avinza, Kadian, OxyContin, Percocet, Dilaudid, Or morph, OxyIR, Methadone, Roxicodone, Hydrocodone, Oxycodone, MSIR, Hydromorphone Actiq, Many other medications under this category.
